ActivStyle makes getting started with home delivery of incontinence supplies easy. Here’s how:

Step 1: Collect Information 

Call us, and one of our caring consultants will check if you’re eligible and get some basic info from you:

• Personal Information – Name, address, phone number, email for contacting you, shipping, and reordering.
• Medical Condition and Diagnosis – information about your condition to recommend the right products.
• Doctor’s Information – Name and contact information so we can get a prescription and other paperwork.  
• Insurance Information – Details about your insurance plan.

Step 2: Verify Insurance 

We’ll make sure your insurance is active and see if your supplies are covered. Then we’ll do the required paperwork for billing.

Step 3: Get a Doctor’s Prescription and Medical Information  

Once we’re sure about your insurance, we’ll ask your doctor for a prescription and any other info they have about your incontinence diagnosis that is required by your insurance plan.

Step 4: Supplies are Shipped to You 

Finally, we’ll call you to verify you still need the supplies and that you authorize us to send them to you. That’s all it takes to get started with ActivStyle.
